I declare that the information given on this form is correct. I understand that giving incorrect or misleading information, or any omission made with the intention of misleading the National Information Technology Agency (NITA), may lead to the rejection of my application and revocation of license, whenever it is discovered. I accept the conditions governing this document.
Data Protection
By submitting this Form, you hereby agree that NITA (“the Agency”) may collect, obtain, store and process your personal data that you provide in this form for the purpose of managing your contract for licensing and for related purposes, such as analysis for management purposes and statutory returns, legal and regulatory compliance and crime prevention. You have supplied us with your personal data and we can therefore process, use and disclose personal data about you as is necessary in compliance with Data Protection Act 2012 (Act 843).
For the avoidance of doubt, Personal Data includes all data defined within the Data Protection Act 2012 (Act 843) including all data you disclosed to the Agency in this Form.
You hereby give your consent to NITA to:
- Store and process your personal data;
- Disclose your Personal Data to the relevant governmental authorities or third parties where required by law or for legal purposes.
